To place an order,
1. Print out the CUYHA Apparel Order Form and fill out your order, or fill in the form on your computer and print the order out.
2. Total up the amount and write a check to "CUYHA" for the appropriate amount.
3. Place order form and check in an envelope and give it to Joe Murphy before, during or after Mite practices. Other Board Members/Coaches may be able to take your envelope and get it to Joe, as well.
4. Let Joe know if other arrangements need to be made.
Note: Orders will be placed on specified dates, to be determined.
